Arrange and/or provide for the psycho/social, emotional and/or financial needs of patients/residents, families and staff in the hospital and outpatient service areas.
Provide all aspects of social services within the hospital and outpatient service areas, as assigned by the Medical-Surgical Director. Services will consist primarily of identifying and meeting the psycho/social, emotional, and/or financial needs of the patients; discharge planning and/or counseling; guidance and support either by direct service or through community resources. Perform other duties as assigned by the Director.

Bachelor’s degree required (from accredited school of social work preferred). Current MN Social Work license (minimum licensure level-LSW). Previous social work experience in a medical setting preferred. Should have knowledge of public assistance programs, community resources, good interviewing techniques, and counseling skills. Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit. Must demonstrate knowledge of the principles of growth and development over the life span. Must be able to assess and interpret data about the patient’s status in order to identify each patient’s age specific needs and provide the care needed by the (neonate, pediatric, adolescent, adult, and/or geriatric) patient group.
Shifts: 40 hours biweekly, Variable days of the week and shift times, call every 4th or 5th weekend.
